Forest Hills School District Photo Release Opt-Out Form

The Forest Hills School District would like to celebrate your student's accomplishments by sharing his/her successes with our schools and the community. For us to do so, we submit press releases that include students' names and photographs to the local media from time to time. We post such information on our district website, district-sponsored publications, social media, and displays at school functions. We intend to be informative and recognize our students' achievements. However, we understand that concerns may arise regarding a student's right to privacy. If you do not grant permission to allow the Forest Hills School District to publish, post, or distribute your student's name and/or photograph or other information related to his/her achievement, please complete this form. Again, this is for those parent(s)/guardian(s) that do not wish to have this information. If the District does not receive an opt-out consent form, the District will assume permission is granted by the parent/guardian.
 

By signing and submitting this form, I formally acknowledge that I do not grant permission for my student’s name and/or photo/image to be published on the school website, its social media platforms, or in the newspaper or other media sources.
