Immunizations

Prior to entry into school at Forest Hills Elementary, a child must be properly immunized. Students in Pre-K must be  immunized against rotavirus, haemophilus, influenza type B (Hib), pneumococcal, diphtheria, polio, pertussis, tetanus,  rubella, rubeola, mumps, hepatitis A and B, varicella (chicken pox) and influenza. Students in Kindergarten must be properly  immunized against diphtheria, polio, pertussis, tetanus, rubella, rubeola, mumps, hepatitis B, and varicella (chicken  pox). 

A physician’s record reporting the dates of these immunizations is required from the parent at the time of  registration of the child. Enforcement of the immunization records as a pre- requisite for school attendance will be in  conjunction with the dictates of district policy and state mandates. Parents must contact the school nurse if their child has a medical, ethical or religious exemption.
