I am confirming the assignment from Forest Hills Junior-Senior High School of a locker that has been given to me for free use. I understand it will be my responsibility to return the locker in normal working condition upon withdrawal from school or completion of the school year. Also, I will not permit other students to utilize my locker as I will be held accountable for all contents of my assigned locker.

Considering the school's permission to use the District's locker, I agree that I will not use the locker for any purpose that would violate a law or any school rule. I further agree to permit the District to open the locker and search its contents in the exercise of its discretion. In exchange for using such a locker, I agree not to hold the school district or its administrators liable for any property loss. Failure to submit this form promptly does not relinquish parent or student responsibility.
