Registering A New Student


Parents/Guardians with school-aged children who are new to the community or have recently moved into the Forest Hills School District are encouraged to register their child(ren) as soon as possible.  The Elementary School enrolls students from Pre-K through Grade 6.
Enrollment information must be completed by the primary parent and/or legal guardian.  If the individual registering the child is not the child's parent, the adult registering the child must provide appropriate, legal documentation to show dependency or guardianship. 

The following documents must be provided when you enroll:  an original birth certificate and current immunization records for your child(ren) at the time of registration, driver's license,  FH Residency Form (see above) and any custody/court documents (if applicable). You will need to complete an enrollment packet for your child as well and show proof of residency from the attached list above.  Forms are available on the link above for your convenience. 

At this time, you may mail the required forms to the school at 547 Locust St., PO Box 290, Sidman PA 15955.


Summer Screening dates for 2020-2021 will be set up at a later date.
