• Field House Rules & Regulations

    This form is required to be signed by the organizational leader in charge of the organization/team using the Field House.  The form lists the rules & regulations which must be followed by all parents and students using the facility.  Once complete, the form is to be given to the Field House monitor or Director of Athletics prior to the first day of use.

  • Field House Waiver

    Any individual wishing to use the Field House must sign and date the form, agreeing to the terms and conditions of using the facility.  Completed forms must be given to the Field House monitor before permission is granted.
